Clinician – Fee for Service
Malta, NY
MSWs with their Limited Permit are eligible for a $2,500 signing bonus!
Licensed Social Workers are eligible for a $7,500 signing bonus!
Must be working 20 or more hours per week to qualify for the bonus
As a Clinician, you will work as part of an interdisciplinary team with the underserved populations in our community by providing clinical counseling to individuals, families and couples in an outpatient mental health clinic licensed by the New York State Office of Mental Health (OMH). You will report to the Director of the Behavioral Health Center. Sessions may be conducted in the office or via Tele-mental Health as clinically indicated.The pay is $40 - $54 depending on licensure.
Responsibilities:
- Manage a caseload of clients aged birth to 65, providing comprehensive assessment, diagnosis, and individual and family therapy services to those assigned to caseload.
- Create initial treatment plans, subsequent treatment plan reviews, progress notes for each scheduled appointment, and other documentation/assessments as indicated.
- Collaborate with on-site prescriber, other medical and mental health providers, schools and any other community supports for client care.
- Attend Behavioral Health Center staff meeting twice a month.
- Attend and participate in Supervision; weekly, bi-weekly or monthly based on licensure level or individual needs.
- Provide referrals and linkages to community supports, resources, and services.
Requirements and Education:
- Master's degree in Social Work or Mental Health Counselor.
- Experience in human services, case management, line staff at a residential program, or any other related direct care within human services, both pre and post master degree, will be considered.
- License and/or Limited Permit eligible, have a limited permit in discipline or Fully Licensed (LMHC/LCSW/LCSW-R), preferred.
- Good standing with the NYS Office of the Professions.

Description
Are you looking to make a difference in the lives of youth and families struggling with behavioral health issues? Do you want to develop and supervise your own team and mentor your staff to improve their clinical skills? As the Team Lead of Home Based Crisis Intervention (HBCI), you will provide clinical supervision, support, and mentorship to your team to help them change lives. The HBCI team provides short-term (up to 6 weeks), intensive intervention to youth and families who are in crisis. We work with parents and caregivers to help them help their children. The short-term format means the work is fast-paced and outcome oriented, which means you and your team will produce meaningful outcomes in a short period of time. You will report to the Chief of Crisis Services and work on-site with travel required.
Responsibilities:
- Provide clinical supervision to the team and be responsible for allocation of staff time; provide training and ensure all team members are kept informed of issues.
- Monitor caseload size and coordinate program intakes and discharges.
- Create rotating call schedule for after-hours / weekend coverage by team members.
- Provide education and outreach to the community about HBCI at Northern Rivers; philosophy, program outcomes, theoretical background etc.
- Develop collaborative relationships with other agencies, schools, hospitals, primary care providers, and local government agencies to help meet the needs of this population so they can achieve and maintain their independence.
- Assist primary clinicians to conduct needs assessments, safety planning and wellness management planning with participants. Connect participants and families with the services and support they need. Provide psychoeducation regarding mental health diagnosis and treatment. Develop treatment using a shared decision-making process with clients.
- Assist primary clinicians to respond to on-call needs of families in crisis and deliver crisis intervention services in home-based environment with flexible scheduling cognizant of family needs, with 24/7 availability nights, weekends and holidays.
- Promote safety of the child, family, and community in the delivery of crisis intervention services.
- Assist primary clinicians to conduct comprehensive assessment of family strengths, values, skills, problems, need and barriers to goal attainment while developing a positive working relationship with the families.
- Collaborate and advocate with formal and information community resources and systems that impact family while teaching family members to advocate for themselves and prepare them for transition and service closure.
Requirements and Education:
- Requires a Master’s degree and at least one year of progressively responsible related experience. Experience in a home-based and/or crisis intervention services program. Candidates with training in the Homebuilder model of crisis intervention are preferred.
- Must be a licensed mental health professional. These include, but are not limited to: Licensed Psychologists, Licensed Clinical/Masters Social Workers, Licensed Marriage and Family Therapists, Licensed Mental Health Counselors, or Licensed Creative Arts Therapist.
- Must have a valid driver’s license.

As a Licensed Clinical Community Service Providers, you will provide Children and Family Treatment Support Services (CFTSS). These mental health services available with New York State Children's Medicaid program, give children (up to age 21) and their families the power to improve their health, well-being and quality of life. This program is intended to offer opportunities to better meet the behavioral health needs of children early on and to prevent the onset or progression of behavioral health conditions. These services strengthen children and their families, and help them to make informed decisions about their care and are provided either at home or in the community. You will report to the Clinical Supervisor and you will work remote/hybrid.
Responsibilities
- Provide Home and Community-based Assessments, counseling, and treatment planning to children and their care givers.
- In Person, in the home, community-based, in office and telehealth to meet the needs of our clients.
- Complete an assessment of needs and recommendations for further medically necessary services, such as rehabilitation services. Services are delivered in a trauma informed, culturally and linguistically competent manner.
- Work with family members, advocates and other salient parties to identify goals of treatment. Provide observations and information to collateral contacts for the purpose of the progress and tracking of the Treatment Plan goals.
- Complete all required documentation for submission to the Supervisor within the required timeframes.
Requirements and Education
- Masters degree in Social Work, Counseling, Psychology or related field.
- 1 year of related experience.
- NYS Professional Licensure/Limited Permit - LMFT, LMSW, LMHC, LCSW, and LCAT.

Job Summary | Endowed Constellation Faculty in Future Chips at Rensselaer Polytechnic Institute We are especially interested in candidates who have a demonstrated ability to perform high impact research in semiconductor technology to address the next generation challenges in computing. Areas of focus are rooted in fundamental science and engineering that drive advances in microelectronics, such as heterogeneous integration, advanced logic, advanced packaging, and power electronics. Job Summary | The Department of Biological Sciences at Rensselaer Polytechnic Institute in Troy, NY invites applications for tenure-track/tenured faculty positions at the Assistant, Associate, or full Professor levels. We seek passionate and innovative individuals with demonstrated potential to establish a strong externally funded research program and contribute to the teaching mission of the department. All exceptional candidates will be considered, and we encourage applications from individuals who will bring a unique research approach and skill set to address fundamental problems in neurobiology, cell and developmental, biology, broadly defined. Successful candidates will complement the department research strengths in cellular neuroscience, circadian biology, molecular motors & cytoskeleton dynamics, muscle physiology & disease, neurodegeneration, protein design, folding, structure & functional dynamics, protein misfolding in human disease, microbiology & host-pathogen interactions, biophysics of extreme environments, and freshwater ecology & climate change. The Institute for Data Exploration and Applications (IDEA) enables an opportunity to partner with faculty that are at the forefront of biological data analytics, enabling the modeling and simulation power to advance the quantitative and systems biology approaches that are widely employed in the department. IDEA and the Center for Computational Innovation provide collaborative opportunities to use the fastest supercomputer found at a private university. The ISE Department has been highly interdisciplinary, with faculty employing methods from optimization, mathematics, statistics, cognitive science, machine learning, and artificial intelligence to model, analyze, and solve complex decision-making problems in highly networked natural and social systems, with applications to critical infrastructure, homeland security, on-demand supply chains, and team performance. This work has been supported by numerous federal, state, and foundation awards. Job Summary | The Department of Industrial and Systems Engineering (ISE) at RENSSELAER POLYTECHNIC INSTITUTE in Troy, NY, is amid a planned multi-year renewal of its faculty and mission. As part of this renewal, we seek applications for the Frank and Lillian Gilbreth Endowed Chaired and Tenured Full Professorship in Industrial and Systems Engineering at Rensselaer. Job Summary | The Lally School of Management at Rensselaer Polytechnic Institute seeks applications for a tenure-track (or tenured) faculty position at the Associate Professor level in Business Analytics/Supply Chain Management starting Fall 2024. Candidates must have a PhD degree from an accredited university in Analytics, Operations Research, Operations Management, Supply Chain Management, or a closely related field, along with demonstrated evidence to produce original and innovative scholarly work with publications in premier business journals. The successful candidate will join the School either as a tenure-track assistant professor, or as an associate professor, based on teaching and research experience and success. The successful candidate will have a demonstrated potential and commitment to effective teaching in our graduate and undergraduate degree programs in supply chain and business analytics, as well as provide service to the School of Management and to Rensselaer. The position requires excellence in research in the area of supply chain and analytics, effective written communication skills, and strong technical skills in areas such as analytical modeling, data management, machine learning and computation. Demonstrated interest in emerging technologies including artificial intelligence and Blockchain and in topics relevant to global shifts in supply chains would be a plus. The successful candidate will have the opportunity to engage with and develop research initiatives aligned both within the Lally School and with the new strategic research initiatives at Rensselaer. Job Summary | The Department of Physics, Applied Physics, and Astronomy at Rensselaer Polytechnic Institute in Troy, NY invites applications for a tenure-track Assistant Professor position in the field of Quantum Computing, Quantum Information, and Quantum Technology, with focus on the fundamental aspects or applications of quantum computation, scalability, and error correction on current and future quantum computing devices. Research areas can include but not limited to quantum entanglement, decoherence, and noise in quantum networks, with applications to quantum cryptography, quantum encryption, and secure quantum communications in noisy environments. Possible related areas would also include studying and exploiting the fundamental connections between Quantum Monte Carlo and Quantum Computing, simulating quantum field theory and many-body theory with a quantum computer (in particular, simulating systems with a “sign” problem), quantum communication networks, exotic quantum dynamics, quantum sensing protocols, exotic quantum phases of matter, and theory on the nature of information at the quantum level. With the anticipated of arrival of the IBM Quantum System One on our campus, this hire will have access to explore and implement some of these challenging problems. This hire would be part of a bold effort at RPI to strengthen and extend our current faculty cluster in Quantum Information, and would interact with our current faculty on quantum optics and photon entanglement, quantum field-theory, quantum fluctuations in networks, as well as with researchers from Computer Science, ECSE, Future of Computing Institute, and with the future hires of the recently announced Quantum Constellation. The position also represents a response to federal priorities expressed in the National Quantum Initiative Act. The successful candidate will establish/maintain and externally funded research program, participate in collaborative, multi-investigator research and research training initiatives, and contribute to the high impact teaching program in the Department of Physics, Applied Physics, and Astronomy. As a Licensed Clinical Community Service Providers, you will provide Children and Family Treatment Support Services (CFTSS). These mental health services available with New York State Children's Medicaid program, give children (up to age 21) and their families the power to improve their health, well-being and quality of life. This program is intended to offer opportunities to better meet the behavioral health needs of children early on and to prevent the onset or progression of behavioral health conditions. These services strengthen children and their families, and help them to make informed decisions about their care and are provided either at home or in the community. You will report to the Clinical Supervisor and you will work remote/hybrid.
Responsibilities
- Provide Home and Community-based Assessments, counseling, and treatment planning to children and their care givers.
- In Person, in the home, community-based, in office and telehealth to meet the needs of our clients.
- Complete an assessment of needs and recommendations for further medically necessary services, such as rehabilitation services. Services are delivered in a trauma informed, culturally and linguistically competent manner.
- Work with family members, advocates and other salient parties to identify goals of treatment. Provide observations and information to collateral contacts for the purpose of the progress and tracking of the Treatment Plan goals.
- Complete all required documentation for submission to the Supervisor within the required timeframes.
Requirements and Education
- Masters degree in Social Work, Counseling, Psychology or related field.
- 1 year of related experience.
- NYS Professional Licensure/Limited Permit - LMFT, LMSW, LMHC, LCSW, and LCAT.

Description
As a Children's Health Home Care Manager, you will engage youth and families through outreach activities until enrollment into the Children's Health Home program. You will coordinate and monitor subsequent services to meet the needs of your members. You will report to a program supervisor, working in a hybrid model. Work will involve a combination of remote, office, and community-based settings. Local travel and personal transportation is required.
Responsibilities:
- Level I - Provides care management services to children with low and medium acuities. Care Manager will provide the required one or two services each month based on acuity and need. One of these contacts is to be in-person with the member.
- Level II - Provides care management services to low, medium, and high acuity members. Care Manager will provide a minimum of two services each month, one of which is to be in person with the member.
- Engage children, youth, and families with outreach and enrollment activities for successful enrollment into a children's health home
- Gather and complete assessments and enter documentation that confirms eligibility and acuity, identifying both strengths and areas of need
- Work w/children, youth, families, and service providers to create a comprehensive Plan of Care
- Identify and engage additional providers (e.g. natural supports, parent advocates, primary care physicians) with youth and families for the execution and continuation of the Plan of Care
- Convene and conduct Interdisciplinary team meetings to review Plan of Care; identifying progress, continued needs, and confirming ongoing eligibility
- Document progress and arrange for transitions between systems, placements, and higher levels of care following changes in eligibility and acuity.
- Use NY State and Health Home information systems to input required Health Home documentation
- Comply with Medicaid and program billing regulations
- Complete all required and recommended trainings
Requirements and Education:
- Level 1 - Requires an Associate's degree in Social Work or a related field, with 2 to 5 years of experience providing direct services to children, persons diagnosed with mental disabilities, developmental disabilities, or substance abuse.
- Level 2 - Requires a Bachelor of Arts or Science degree, and two years of experience providing direct services to children, persons diagnosed with mental disabilities, developmental disabilities, or substance abuse.

- Participate in developing an active recruitment plan for recruiting foster parents
- Handle inquiries and applications from potential foster parents
- Facilitate all orientation meetings with potential foster parents
- Complete all paperwork for the certification process
- Prepare and present NTDC training for potential foster parents
- Assess families’ ability to foster children as it relates to the 14 Characteristics of Successful Parents who Foster or Adopt
- Complete home studies for all recommended foster homes
- Conduct safety assessments to assure the health and safety of foster homes
- Facilitate agency pre-certification trainings
- Maintain certified foster family records
- Handle the recertification process, including development of yearly reauthorization narrative with each family, assessing strengths and needs of the foster home
- Provide ongoing training and support for certified parents in conjunction with the foster care team
- Check with certified families on quarterly basis to monitor strengths and needs of the foster home
- Consult with Program Coordinator on homes and placement of children
- Coordinate annual holiday party and a foster parent appreciation event as retention efforts
Requirements and Education:
- Requires a Bachelor’s degree in human services, social work, sociology, psychology, education or related field
- At least two years of related experience
- Requires a valid New York driver’s license
